本研究では,認知症高齢者に対し,精神療法的ないし心理・社会的アプローチを行うことを非薬物療法として捉え,認知症の中核症状である認知機能に焦点を当てて,コラージュ療法の非薬物療法としての意義について臨床的介入の結果から分析・考察した。対象は,老人保健施設の認知症エリアに入所中の12名である。月2回,6ヶ月間,音楽療法とコラージュ療法を併用した活動を行い,その効果をHDS-Rおよびコラージュ観察評価スケールを用いて評価した。その結果(分析対象は9名),ADの2名を除く7名中6名は介入後のHDS-R得点が増加した。また,対人交流の活性化や活動への注意・関心の高まり,感情の安定化が認知機能の維持・改善に寄与することが示された。コラージュ療法の作業過程と制作された作品の感想から満足感や達成感,回想などによる感情の浄化と安定が図られ,行動の変容に繋がるなど非薬物療法として有意義であることが示された。 |

In this study, we considered psychotherapy or psycho-social approaches for elderly people with dementia as a form of non-drug therapy. We analyzed the effects of a clinical intervention using collage therapy with a focus on the loss of cognitive function that is the central symptom of dementia. The subjects were twelve people (the analysis subjects: nine) living in the dementia section of health service facilities for the elderly. Activities using music therapy together with collage therapy were conducted twice a month over a period of six months, and the effect was evaluated using HDS-R and the collage observation assessment scale. Results indicated that, excluding AD patients, six out of seven had an increased HDS-R score after intervention. Moreover, it was shown that activation of interpersonal exchange, increased attention and concern, and stabilization of emotion contributed to maintenance and improvement of cognitive function. From the work process and impressions of the work created it can be said that significant aspects of collage therapy as a non-drug therapy included feelings of satisfaction and achievement, catharsis (purification of feelings) connected with recollection, and behavioral changes. |
